Note: You cannot renew an expired license in the portal. Please use the reinstatement application found below under REINSTATEMENT.

If your license has been expired less than 1 year, please complete this application and mail to the office with your reinstatement fee.
If your license has been expired for more than 1 year, please complete the initial application found at the top of this page.
