Applications for Nurse Agencies
This resource page provides applications for licensure, required forms to be completed and submitted with applications, education or certification requirements for renewal of a license.
Initial Registration
To register as a Nurse Agency with the Office of Professional Licensure and Certification, an individual or entity must complete the following:
- Universal Licensure Application
- Nurse Agency Supplemental Application
- Fee- No Fee Required
- If applying on behalf of an entity
- Copy of document authorizing individual signing the application to do so on the entity’s behalf;
- Confirmation from New Hampshire Secretary of State’s Office that the entity is in good standing and authorized to do business in New Hampshire
