Please submit the “Candidate for Licensure: Supervision Agreement” to apply for a Conditional License. Please note that the fee to apply for a supervision agreement only is $25.00. The fee to apply for a conditional license is $285.00. (Plc 1002.29) as well as a mandatory Professional Health Program (PHP) fee of $28.00 to be paid at the time of application.
