NH State Laws/Rules Regarding Reporting Changes - Pharmacy Technicians

Change in Registration Information for All Pharmacy Technicians.

NH RSA 318:26-a
Change in Name, Employment, or Residence. – Any pharmacist or pharmacy technician who changes his or her name, place or status of employment, or residence shall notify the board in writing within 15 days. For failure to report such a change within 15 days, the board may suspend the pharmacist's license or the pharmacy technician's registration.
Reinstatement shall be made only upon payment of a reasonable fee as established by the board.

(a) The person to whom a pharmacy technician registration has been issued shall, within 15 days of a change of address or location of employment, notify the board of such changes.

(b) The notice shall contain:

1. Legal name of registrant;

2. Address, phone number, and personal email address of the registrant,
including old and new, if applicable;

3. Registrant’s registration number;

4. Name of the pharmacy where employed including former and current, if
applicable; and

5. All new violations of law, convictions, fines, disciplines or any registration,
certification, or license revocations for violation of pharmacy-related drug laws or
regulations in this or any other state.

(c) Failure to comply with Ph 805.01 shall constitute misconduct under RSA 318.
