NH Alcohol and Drug Counselor Licensure Survey

The NH Department of Health and Human Services, in partnership with the NH Board of Licensing for Alcohol and Other Drug Use Professionals, is collecting key practice and capacity data from licensed alcohol and drug counselors during the license renewal cycle.

Pursuant to RSA 330-C:9-a, licensees are "required, as part of the license renewal process," to complete the survey. However, providers may also opt out of participation by completing the opt-out form found on the NH Health Professions Data Center website.

The individual data collected will be confidential and protected. Aggregated data will be used for statewide healthcare workforce planning, assessment, and emergency preparedness. Data summaries will be released by the Health Professions Data Center, annually, for public consumption.

The survey will be available to complete on the renewal website during the license renewal process.

Please contact the Health Professions Data Center at NHLADCSurvey@dhhs.nh.gov with questions or concerns. Do not contact the board with any technical questions related to the survey.
